Opting Out and Opt Out Revocation

At MyHealth Access Network, your privacy is a top priority. We use industry-leading safeguards, including encryption and secure connections, to protect your health information. MyHealth also complies with all federal and state privacy laws, including HIPAA.

If your healthcare provider participates in MyHealth, your health information is securely shared through our network to support better, more coordinated care. However, as a patient, you have the right to opt out at any time. You can also change your mind later and choose to opt back in.

To opt out or revoke a previous opt-out:

  • Step 1: Print and complete the appropriate form below.
  • Step 2: Submit the form to your participating healthcare provider, who will verify your identity and can deliver it to us immediately.

  • Step 2: Have the form notarized to verify your identity, then mail to us,

Once your request is processed, MyHealth Access Network will send you a confirmation letter.
