LifeShare Network of Oklahoma

In organ and tissue donation, every minute matters. As Oklahoma’s Organ Procurement Organization, LifeShare faced delays accessing the clinical information needed to evaluate donors, coordinate with hospitals, and match organs with recipients. Through real-time access to statewide health data via the MyHealth Provider Portal, LifeShare accelerated donor evaluation, improved coordination across care teams, and reduced wait times—helping ensure more lives are saved and the gift of donation is maximized.


Oklahoma’s Office of the Chief Medical Examiner

For the Oklahoma Office of the Chief Medical Examiner, immediate access to decedent medical records is mission critical. CREOKS Behavioral Health Services

Behavioral health providers often lack real-time visibility into their clients’ physical health, creating gaps in care coordination. CREOKS uses the MyHealth Health Information Exchange to bridge behavioral and primary care, giving care managers access to comprehensive medical histories, medication lists, and lab results. VIPcare

VIPcare specializes in care for older adults and supports a complex patient population with frequent emergency department visits and hospitalizations. To improve care coordination and reduce rehospitalization risk, VIPcare implemented MyHealth’s Care Fragmentation Alerting (CFA) service. CFA provides timely notifications when patients interact with the emergency department, enabling care teams to quickly follow up, address barriers such as transportation, and redirect patients to appropriate outpatient care. This proactive approach helps reduce unnecessary emergency room use, avoid duplicative testing, lower patient costs, and improve overall outcomes for elderly patients.
